Preventive care is a key component of comprehensive primary care. The goal of our preventive check-ups is to identify health risks early, prevent illness, and maintain your quality of life in the long term. Disease management programs
For patients with chronic illnesses, we offer structured and regular preventive care within the framework of Disease Management Programs (DMPs). The goal is a stable state of health and the prevention of secondary illnesses.
Our support includes:
- Regular follow-up examinations every three to six months
- Targeted blood tests and medical check-ups
- Individual adjustment of therapy based on current findings
- Conditions treated: Type 2 diabetes mellitus, coronary heart disease (CHD), COPD, bronchial asthma

All patients of our general practice in Hamm-Heessen can take part in preventive check-ups. The services depend on age, individual health status, and any pre-existing conditions.
Patients with statutory health insurance are generally entitled to a health check-up every three years. If enrolled in a GP-centered care program (HZV), a check-up is possible every two years. We are happy to advise you personally at our practice in Hamm.
The check-up includes medical consultation, a physical examination, and—depending on the case—blood tests. Among other things, cardiovascular risks, metabolic values, and other relevant health factors are assessed.
Many preventive services, including check-ups and DMP programs, are covered by statutory health insurance. We will inform you transparently in advance about any additional individual services.
DMPs are structured treatment programs for chronic conditions such as type 2 diabetes, coronary heart disease (CHD), COPD, or bronchial asthma. The goal is regular monitoring, stable treatment, and prevention of complications—close to home at our practice in Hamm-Heessen.
